  • Auto opening doors to waiting area
  • Reception desk has wheelchair access
  • Disabled Car Parking and step free access from car park
  • Translation services available including British Sign Language
  • Accessible toilet facilities

Infection Prevention and Control Annual Statement

Infection Control Annual Statement 2023

Purpose

This annual statement will be generated each year in accordance with the requirements of The Health and Social Care Act 2008 Code of Practice on the prevention and control of infections and related guidance.  It summarises:

  • Any infection transmission incidents and any action taken (these will have been reported in accordance with our Significant Learning Events Procedure)
  • Details of any infection control audits undertaken and actions undertaken
  • Details of any risk assessments undertaken for prevention and control of infection
  • Details of staff training
  • Any review and update of policies, procedures and guidelines

Infection Prevention and Control (IPC) Lead

Hyde Park Surgery has one lead for Infection Prevention and Control: Sister Rekaya Jeffers, Practice Nurse.

The IPC Lead is supported by Helen Craggs, Assistant Practice Manager.

Infection Transmission Incidents (Significant Learning Events)

Significant Learning events (which may involve examples of good practice as well as challenging events) are investigated in detail to see what can be learnt and to indicate changes that might lead to future improvements.  All learning events are reviewed in the practice staff meetings and learning is cascaded to all relevant personnel.

In the past year there have been no learning events raised that related to infection control.

Infection Prevention Audit and Actions

The Annual Infection Prevention and Control audit for Hyde Park surgery was completed by Rekaya Jeffers and Helen Craggs in December 2023.

As a result of the audit, the following things have been changed or are currently being reviewed:

  • Due to retirement of a practice nurse, Rekaya Jeffers is now the lead nurse for infection control and all policies and documents have been updated.

 Risk Assessments

Risk assessments are carried out so that best practice can be established and then followed.  In the last year the following risk assessments were carried out:

Immunisations: As a practice we ensure that all of our staff are up to date with their Hepatitis B immunisations and offered any occupational health vaccinations applicable to their role (ie MMR, seasonal flu).

Cleaning specifications: We have a cleaning specification and frequency policy which our cleaners and staff work to.

Training

All our staff receive annual training in infection prevention and control and hand hygiene.

All hand washing facilities for patients and staff have a pictorial display of correct hand washing techniques.

Policies

All Infection Prevention and Control related policies are in date for this year.

Policies relating to Infection Prevention and Control are available to all staff and are reviewed and updated annually by Rekaya Jeffers and Helen Craggs, and all are amended on an on-going basis as current advice, guidance and legislation changes.  Infection control policies are available to staff for reading on the shared drive.

It is the responsibility of each individual to be familiar with this statement and their roles and responsibilities under this.

The Infection Prevention and Control Lead and the IPC support lead are responsible for reviewing and producing the Annual Statement.

Publication

A copy of this Annual Statement will be uploaded to our practice website, and we will notify patients of any seasonal or other outbreaks that they may need to be aware of via posters in the surgery and on our website.

In May 2018 the Data Protection Act 1998 has been replaced with the General Data Protection Regulations. Please view our Privacy Notice, Patient Leaflet and FAQs. These documents will be updated as the new regulations are clarified with regard to health.

February 2019 – Our practice is collaborating with NHS Business Services Authority (NHSBSA) which is providing NHS Numbers for those patients that may be suitable for electronic repeat dispensing prescriptions. The information is obtained from NHS prescriptions submitted to the NHSBSA. A member of our clinical team will contact these patients soon to explain the benefits of the service.

Data Protection Compliance Statement (Privacy Notice) – STAFF

GP Earnings

All GP practices are required to declare the mean earnings (average pay) for GPs working to deliver NHS services to patients at each practice.

The average pay for GPs working in Hyde Park Surgery in the last financial year was £70,721 before tax and National Insurance. This is for 7 part time GPs who worked in the practice for more than six months.

Privacy Notice – Recording Telephone Calls

 

 Plain English explanation

The surgery has the ability to record telephone calls to protect patients and staff and other health workers. Patients are protected by our having a record of our conversations with you, staff and other health workers are protected from potential abuse. All calls to and from the surgery may be recorded. All calls are deleted after 3 months. We also occasionally use recordings for staff training and quality control.

When you register with us we will make this clear to you and we will also make this clear to you via our web site and other sources of information, such as a recorded message that will be played before inbound calls are answered.

Calls, or transcripts of calls, audio or audio-visual recordings or elements of the discussion you have with the clinicians that contain clinical information may be added to your medical records, but this will be clarified with you at the time.

The recordings are stored securely on a system provided by Redcentric and are protected through the company’s GDPR Policy.

These recordings will not usually be shared outside the practice.

If we hold recordings that have not been deleted you can ask for copies by putting a request to the Practice in writing via a Subject Access Request.

We are required by Articles in the General Data Protection Regulations to provide you with the information in the following 9 subsections.

4) Lawful basis for processing The processing of personal data in the delivery of direct care and for providers’ administrative purposes in this surgery and in support of direct care elsewhere is supported under the following Article 6 and 9 conditions of the GDPR:

Article 6(1)(e) ‘…necessary for the performance of a task carried out in the public interest or in the exercise of official authority…’.

Article 9(2)(h) ‘necessary for the purposes of preventative or occupational medicine for the assessment of the working capacity of the employee, medical diagnosis, the provision of health or social care or treatment or the management of health or social care systems and services…”

We will also recognise your rights established under UK case law collectively known as the “Common Law Duty of Confidentiality”*

5) Recipient or categories of recipients of the processed data Necessary data will be shared with Health and care professionals and support staff in this surgery. Clinical data or records of consultations may be transcribed or appended to the records we hold on you and may thence be shared at hospitals, diagnostic and treatment centres who contribute to your personal care. Actual recordings will not be shared with anyone outside the practice. Please see our Privacy Notice for Direct Care. The actual recordings are stored on the telephone system and after a designated period of time will be deleted. You will be informed if the recording is to be stored for longer than is set out in this Privacy Notice and you will informed the reasons for such.
6) Rights to object You have the right to object to some or all the information being processed under Article 21. Please contact the Data Controller or the practice. You should be aware that this is a right to raise an objection, that is not the same as having an absolute right to have your wishes granted in every circumstance
7) Right to access and correct You have the right to access the data that is being shared and have any inaccuracies corrected. We can provide copies of recordings if they have not been deleted. There is no right to have accurate medical records deleted except when ordered by a court of Law.
8) Retention period We will keep recordings for the period of our contract with our provider. Clinical data transcribed from your telephone or other electronic consultations may become part of your clinical record and is retained according to relevant rules and regulations, see Privacy Notice on Direct Care.

* “Common Law Duty of Confidentiality”, common law is not written out in one document like an Act of Parliament. It is a form of law based on previous court cases decided by judges; hence, it is also referred to as ‘judge-made’ or case law. The law is applied by reference to those previous cases, so common law is also said to be based on precedent.

The general position is that if information is given in circumstances where it is expected that a duty of confidence applies, that information cannot normally be disclosed without the information provider’s consent.

In practice, this means that all patient information, whether held on paper, computer, visually or audio recorded, or held in the memory of the professional, must not normally be disclosed without the consent of the patient. It is irrelevant how old the patient is or what the state of their mental health is; the duty still applies.

Three circumstances making disclosure of confidential information lawful are:

  • where the individual to whom the information relates has consented;
  • where disclosure is in the public interest; and
  • where there is a legal duty to do so, for example a court order.

 

Training

Information about when we close for training and about the surgery being a training practice

Staff Training

Please be aware that we will be closed for staff training one Thursday afternoon every month. During this time the NHS 111 will cover the surgery if you require urgent medical assistance

Training Practice

The practice is committed to the further training of health professionals. These include medical students, nurses and GP trainees. Staff receiving training are fully supervised but if you prefer not to receive care from someone in training please say and we will honour your preference.

Why do we sometimes charge fees?

The NHS provides health care free of charge, but there are exceptions.

It is important to understand that GPs are self-employed and offer their services to the NHS and they have to cover their costs eg. staff, buildings, heating, lighting, etc. The NHS covers these costs for NHS work, but for non-NHS work.

The British Medical Association (BMA) suggests fees for non-NHS work which is not covered under a GP’s NHS contract. However, these fees are guidelines only, not recommendations, and a doctor is not obliged to charge the rates suggested.

Why can it take a long time to complete a form?

Time spent completing forms and preparing reports takes the GP away from the medical care of his or her patients. Most GPs have a very heavy NHS workload and this paperwork is done in around the patient care.

When a doctor signs a certificate or completes a report, it is a condition of remaining on the Medical Register that they only sign what they know to be true. In order to complete even the simplest of forms, therefore, the doctor might have to check the patient’s entire medical record. Carelessness or an inaccurate report can have serious consequences for the doctor.

Named Accountable GP

Every patient at the practice is allocated a named, accountable GP

 

Every patient at the practice is allocated a named, accountable GP. This ensures you have a ‘usual’ GP as a point of contact and to assist with continuity of care. You should always try and see your usual GP, especially for ongoing problems, however we understand this is not always possible which is why there are no restrictions on seeing other doctors.

It is possible to change your named GP at any time, please contact reception who will be happy to help.

Your rights to information

In addition to accessing the information identified in the Publication Scheme, you are entitled to request information about the practice and the partners under the NHS Openness Code 1995.

The Freedom of Information Act 2000 recognises that members of the public have the right to know how public services are organised and run, how much they cost and how the decisions are made.

From January 1st 2005 General Practitioners have been obliged to respond to requests about information that they hold, that is recorded in any format. These rights are subject to some exemptions, which have to be taken into consideration before deciding what information can be released.

Under the General Data Protection Regulations, you are also entitled to access your clinical records or any other personal information held about you and you can ask at Reception.

All information at Hyde Park Surgery is held, retained and destroyed in accordance with NHS guidelines. Our commitment to publish information excludes any information, which can be legitimately withheld under the exemptions set out in the NHS Openness Code or Freedom of Information Act 2000. Where individual Classes are subject to exemptions, the main reasons are for the protection of commercial interests and personal information under the General Data Protection Regulations. Who We Are

Details of the practice, organisational structures and key personnel can be found in the practice profile in our patient information folder. The practice adheres to the National General Medical Services contract. The contract is with the NHS England.
