Click on the map and enter your postcode in the search box – if your address is in our area the area will be shaded purple (zooming out can make this clearer). Please ring the surgery on 0113 2001090 if you need assistance with this.

How to register as a new patient

Please complete our online registration form. Please ensure you are use your full name, date of birth and full address in Leeds (including Flat number, Block Number if applicable). We prefer 2 forms of identification for each person, one of which should be photo ID and one proof of address (eg utility bill) but please contact the surgery if this is an issue ( see Safe Surgeries information)

New patients will need to complete an initial health questionnaire.

Register as a New Patient

We only accept registration forms online unless you are unable to complete an online form (visit or ring the surgery if this is the case). Complete the form online and a member of the admin team will contact you within two weeks to confirm that you live in the practice area and that we are able to accept the registration and give you more information. Overseas Visitors

If you are an overseas visitor to the UK there may be a charge to see the doctor or nurse. For further information, please contact reception.

Updating your Details

Please let us know if you move address, change your phone number(s) or email. You can do this in person or if you are registered for online services you can do this via the patient portal or your NHS app

Temporary Residents

If you are visiting the area for work, family reasons, holiday or studies, you can register temporarily (from 16 days to 3 months). You will still remain registered with your permanent GP back at home.

To register as a temporary resident the place you are staying needs to be within our inner catchment area. After three months you will have to apply to register with that surgery as a permanent resident.

We will pass details of any treatment you have to your GP, who will add the information to your medical records.
