eConsultation Forms

The aim of an eConsultation is to help you get things done without having to come in to the surgery. This is not for urgent matters. You are able to ask a question e.g. about medication etc. You are able to complete a registration form online but registration is not guaranteed until we have checked that you live within our practice area.

eConsultations are for such things as getting advice from a GP, complete admin tasks or arrange to see other professionals or services without always having to see the GP first.

Think of it as an email but the topic of the email and the information you provide is guided by filling in one of the online forms.

Once completed the form is then sent through to the reception team, who will assign the form to the most appropriate member of the team. This is not for urgent advice and it may take 2 days for the surgery to respond to you.

Once the form has been read and a decision reached, you will contacted by a member of the team informing you of the next step.

Use this form to request a non-urgent appointment with a GP or to ask a question. Please note that due to Coronavirus all appointments and contacts with and by the surgery will be by telephone.

Queries and questions about your medication can be submitted to the surgery on the form below. This is not for urgent matters and it may take 2 days for the surgery to respond to you

Registration form

Due to the Coronavirus (COVID-19) pandemic we are able to accept registration forms online. Complete the form online and a member of the admin team will contact you within 2 working days to confirm that you live in the practice area and that we are able to accept the registration and give you more information.

Request sick note (Med3) – Repeat sick notes can be requested and your request will be passed to your usual GP. Please note that sick notes can’t be forward dated, this service takes 48 hours and the sick note will be sent as an attachment to a text message therefore ensure that you add your up-to-date mobile number.
